What To Expect During Your Initial Visit 

​What to Expect

The initial psychiatric evaluation is a comprehensive 60- to 90-minute appointment designed to understand the full picture of a client’s mental health. During this time, we explore your symptoms, medical and mental health history, lifestyle factors, and any emotional or behavioral concerns to help establish an accurate diagnosis and create a personalized treatment plan.

At the heart of our assessment process is a caring, non-judgmental approach. Lauren Gold and Chantel Rusz strive to create a safe, supportive environment where you feel heard, respected, and empowered to take an active role in your care. Whether the next steps include medication, therapy, or holistic options, we work together to choose what’s best for your unique needs and goals. Initial Assessments are conducted via telehealth. In-person appointments are also available upon request for those who prefer them.

Getting Ready for Your First Appointment


To help us provide you with the best care possible, please bring the following items to your first visit:

Photo Identification

A valid driver's license, state-issued ID, or passport to confirm your identity.

Medical Background

Any documents or records that detail your medical history and past treatments. This helps us understand your overall health more completely.

Medication List

A current list of all medications you're taking, including prescriptions, over-the-counter drugs, vitamins, and herbal supplements. Please include any medications you've taken in the past as well.

Mental Health Records

If available, bring documentation of any previous psychiatric evaluations or treatments. This can include reports from therapists or psychiatrists, prior diagnoses, and notes on any psychiatric medications you've used.

Insurance Information

While we do not bill insurance directly, bringing your insurance card may be helpful if you require labs or referrals to providers who do accept insurance.

Form of Payment

We accept debit and credit cards. Please bring a payment method with you to the appointment.

Completed Forms (If Available)

If you received intake forms ahead of time, bringing them filled out can help save time. If not, please arrive 15 minutes early to complete them before your session begins.

Vital Signs 

Please bring your vital signs (Weight, Height, Heart Rate, and Blood Pressure) to each appointment. This information helps us provide the best possible care.

Family Involvement in Care

Patients Under 18

A parent or legal guardian must accompany minors to every appointment. This ensures that guardians stay informed and actively support the treatment process.

Adult Patients

While not required, we encourage adult patients to consider bringing a family member to their first appointment. A loved one can often offer valuable insights that support a more complete understanding of your situation and improve treatment planning.

Involving family members fosters open communication, encourages collaboration, and helps build a stronger foundation for healing and long-term well-being.​​
